Formal written consent forms are to be used for all intralesional steroid injections; for gynecologic colposcopy, biopsy, and cryosurgery of the cervix; and for dermatologic minor surgical procedures done at the Student Health Service. Appropriate consent forms are available for use in each clinic.

Verbal consent for all other procedures should be documented in the chart note by the practitioner performing the procedure. If the practitioner desires to use a formal written consent form, he/she may choose to do so. A supply of appropriate forms are available in the clinics.

All consents are currently scanned into the EMR.
