It is the policy of the Student Health Service that all employees sign the following confidentiality statement at the beginning of employment at Student Health.
“It is a policy of Student Health Service that all charts, reports, records, and conversations regarding care of patients of the Student Health Service are kept confidential and are not discussed outside of the department. I understand this policy and agree to abide by it.”
In addition, Medical Records personnel and others to whom it applies signs this statement:
“Further, I understand that I am not to review or otherwise investigate charts except to specifically obtain information for processing purposes.”
